Contact your VA Medical Center
Contact your local VA Medical Center to request a mobility evaluation. Request an appointment with Physical Therapy (PT) or Occupational Therapy (OT) for mobility evaluation.
Tip: When booking, say you’re interested in being evaluated for a VELA Independence Chair, and ask to meet with a provider who handles durable medical equipment (DME) or assistive technology assessments.
Get Your Evaluation
Your VA clinician will assess your mobility needs. If the VELA Chair is medically appropriate, they’ll write a clinical justification.
VA Orders your VELA Chair
Once the clinician completes the evaluation and writes a clinical justification, the VA’s prosthetics or purchasing department will order the chair for you.
The VELA Chair is available on the Federal Supply Schedule (FSS), which simplifies and speeds up the purchasing process.
